To become a new patient of NorthEast Internal & Integrative Medicine, please complete our online forms in Adobe Acrobat. Once complete, simply print, sign, and bring them to the first appointment or consultation.
Insurance
While Integrative Medicine is growing in popularity among health consumers and acceptance in the traditional western medical community, many insurance companies do not provide coverage for these services. At this time due to insurance limitations, the consultations will remain on a fee for service basis. However we will be actively working with the insurance companies to change this reimbursement status, to one which is more beneficial for the patient.
Fees
As insurance currently does not provide coverage for these services, all fees are due at the time of service. We accept cash, check or credit cards. If there are special circumstances surrounding your financial obligations, please speak to a front desk staff prior to your appointment about establishing a financial plan that meets your needs. We ask that all our patients sign and submit our financial policy statement prior to the initial appointment.
All fees are due at the time of service. All balances not paid within 30 days will be assessed a late fee of $25. Balances 60-days past due will be turned over for collection. A $25 returned check fee will be assessed for insufficient funds.
